Student COVID-19 Vaccination On-Site Clinic: Pre-registration Deadline May 7, 2021 at 5pm
BHS will be hosting an on-site COVID-19 School Vaccination Clinic through Wal-Mart. The clinic will be for students age 16 or older and will be run with the same safety protocols as other vaccination clinics run across the state.
Clinic date/time to be determined, but will most likely be held during the school day. We will notify you as soon as the clinic is scheduled.
Parental Consent for Vaccination of 16 and 17 Year Old Teens: CDPHE Updated Guidance
· Children ages 16 and 17 who are not emancipated must have parental or legal guardian consent for any vaccine.
· A parent or legal guardian must accompany the minor to receive the vaccine unless the vaccine administration occurs in a physician’s office, school-based or school-associated clinic, or similar setting. In the specific settings noted, 16- and 17-year-olds may now be vaccinated without parental presence.
· The vaccine will be Pfizer
· COVID-19 Vaccines are free, however, there is an administration fee for the provider (Wal-Mart) of $15. This will be billed directly to your insurance.
Students who are 18 years old, or an emancipated minor, may complete their own pre-registration and consent form. A consent form will be provided on-site on the day of the clinic.
